- Death Report From Vcdc/Gaon Burah/Town Committee (Any)
- Form 10 Non-Availability Certificate
- Gaonburah/Vcdc/Residential Certificate of Applicant And Deceased
- Court Affidavit
- Deceased Person ID Proof (If Any)
- Applicant Aadhar/Voter/Pan (Any)
- Current voter list/voter certify copy (Any)
- Up-to-Date Land Revenue Receipt
- Legacy Data Or Voter Certify Copy Before 1971
- Permission Copy
N.B.:- If there is a delay of more than 21 days in initiating the application process for a birth or death certificate, a permission copy will be required
