Documents require for permission of delayed Death Registration/Certificate

Here is the list of all the documents required to apply for a permission copy
  • Death Report From Doctor & Samshan
  • Death Report From Vcdc/Gaon Burah/Town Committee (Any)
  • Form 10 Non-Availability Certificate
  • Gaonburah/Vcdc/Residential Certificate of Applicant And Deceased 
  • Court Affidavit
  • Deceased Person ID Proof (If Any)
  • Applicant Aadhar/Voter/Pan  (Any)
  • Current voter list/voter certify copy (Any)
  • Up-to-Date Land Revenue Receipt
  • Legacy Data Or Voter Certify Copy Before 1971
  • Permission Copy

N.B.:- If there is a delay of more than 21 days in initiating the application process for a birth or death certificate, a permission copy will be required
